Job Description:

Citizens Memorial Hospital (CMH) is recruiting a Clerk for our Care Coordination Department.

A typical day will include: Serving as the in clinic resource for facilitating care needs outside the clinic; working closely with patients, providers, and other staff to schedule appointments and obtain authorization for primary or specialty appointments and diagnostic testing; tracking information regarding appointments and admissions outside the medical home, and communicating status or delays to the care team. The clerk will also ensure that the record of care that occurs outside the EMR is received and added to the patient's electronic medical record (EMR).

The position is full-time, days. Monday through Friday 8am-4:30pm.
