Job Description:
The Registered Nurse Case Manager plans, organizes, and directs
hospice care, focusing on community health education and resource
utilization.
Conducts comprehensive patient and family assessments to develop
and update individualized care plans.
Provides professional nursing care, including medication
administration, health education, and support for activities of
daily living.
Coordinates with physicians and the interdisciplinary team to
ensure continuity and quality of care.
Communicates patient needs, documents care, and educates families
on self-care techniques.
Participates in on-call duties, supervises ancillary staff, and
engages in ongoing professional development.
Requires nursing licensure, recent acute and home care experience,
and a valid driver--------s license.
