Summary Full time Case Management allows for flexible scheduling to promote work/life balance. Weekends every 6 weeks, No On Call. Job Summary Provides in home evaluation, direction and application of skilled Nursing Services to complement the patient's overall Plan of Care. Job Requirements Education Required: Graduate of an approved nursing program Experience Preferred: Minimum one (1) year experience as professional nurse within the last three (3) years. Skills Ability to provide reliable transportation as required by nature of position.
