This office services our adult and geriatric clients on a per visit basis in territories throughout Christian, Douglas, Greene, Howell, Ozark, Polk, Stone, and Taney counties. If you enjoy providing patient care within a home setting and are inspired to uphold our promise of quality care, this is your opportunity to make a difference!As a Hospice Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home. Responsibilities for a Physical Therapist (PT) include: Identifies safety risks and uses professional judgment in progressing patients through plan of treatment. Evaluates and recommends/performs Physical Therapy interventions for symptom control/palliation. Provides office consultations to the nurse of hospice IDG. Meets agency standards for timeliness and completeness of documentation and responds of referrals in a timely manner. Provides physical therapy, for hospice patients when requested by the physician and hospice IDG. Instructs patients/caregivers in the home exercise program for continued palliation between visits and in the care and use of equipment designed to accommodate for physical disabilities. Qualifications for a Physical Therapist (PT) include: A current Missouri Physical Therapist - PT license. A minimum of one year of recent work experience as a Physical Therapist - PT. Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills.
