Works as part of the home health care team to ensure patients receive timely admission to home health. Creates a specialized plan of care in collaboration with patient and family with a focus on improving the health and safety of the patient

  Must be minimum 18 years of age per state guidelines.
  Registered Nurse (RN) license required
  Home Health experience preferred but not required.
