*Overview*We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a passion for helping individuals in need and possess a strong background in social work and crisis intervention.*13 week contract ; $54.50 blended hourly rate ; 5x8 day shift**Current licensure in a clinical discipline in the state of MO (RN, RT, SW preferred)**Associates Degree in a healthcare field (Bachelors degree preferred)**Working knowledge of the insurance industry and government reimbursement**2 years experience in Case Management and Discharge Planning in a Rehab Care setting**Duties*- Provide case management services to clients, including assessing needs, developing care plans, and monitoring progress- Conduct intake interviews and assessments to determine client eligibility for services- Collaborate with other agencies and professionals to coordinate services for clients- Offer counseling and support to individuals facing challenges such as addiction, mental health issues, or housing instability- Advocate for clients' rights and access to resources such as workforce development programs, Section 8 housing, and Medicare benefits- Utilize motivational interviewing techniques to empower clients to make positive changes in their lives- Mentor clients in developing life skills and achieving personal goals*Skills*- Strong background in social work, behavioral health, or related field- Knowledge of child welfare systems and best practices- Experience in crisis intervention and addiction counseling- Familiarity with Medicare regulations and benefits- Proficiency in motivational interviewing techniques- Excellent communication and interpersonal skills- Ability to work effectively in a team environment- Strong organizational skills and attention to detailIf you are passionate about making a difference in the lives of others and possess the skills needed to excel in this role, we encourage you to apply for our Case Manager position.Job Type: ContractPay: $54.00 per hourExpected hours: 40 per weekBenefits:* 401(k)* Dental insurance* Health insurance* Life insurance* Retirement plan* Vision insuranceSchedule:* 5x8* 8 hour shift* Day shiftAbility to Commute:* Springfield, MO (Required)Ability to Relocate:* Springfield, MO: Relocate before starting work (Required)Work Location: In personby Jobble
